Having worked on successful and "challenged" waterfall, iterative, and agile projects, the reality is that the longer projects take, the longer you have to wait to recognize value.
Why do we prefer agile approaches? At it simplest, because agile projects recognize value faster.
New and changing requirements are two of the most commonly cited reasons projects fail or are over budget.
Fixing these issues tends to focus on how to prevent change or spending more time documenting requirements.
The real question is,
"Why are the requirements missing or wrong"?
There are quite a few reasons, but at the core is the
recognition that people are not sure what they want. When asked about a new product, most people are not sure exactly what they want and need
unless they can see it (a bit of the chicken or the egg).
Changing requirements are an issue on every software development project and any other project that is developing a new product.
The issues however have more to do with trying to lock in all the requirements early, instead of embracing change.
Requirements on a software project will change and be added. That is a fact. So
figuring out how to leverage change for success is the larger question!

Implementing only one or two agile concepts is not going to "fix everything." It may improve your success incrementally or not.
We see the
critical turning point in increased value when customers begin delivering working software and value every thirty days.
When your customer has been involved in the process and can see and use working software 30 days after that first meeting - that's really when everyone gets it.
